At our Sparta office, we treat the jaw joints, the bite, and the airway as one connected system, because strain in one usually shows up as symptoms in the others. A narrow airway can pull the lower jaw into a strained position during sleep to keep air moving. Clenching and grinding, in turn, often go with disturbed sleep.

Meet Dr. Zoltek
Dr. Ewa Zoltek, DMD, DICOI, has practiced for more than 18 years and is the youngest dentist in the US to earn Diplomate status with the International Congress of Oral Implantologists.
Dr. Zoltek uses advanced diagnostic technologies to evaluate jaw function, bite relationships, muscle activity, and airway health. She built her practice in Sussex County around treating dental symptoms as whole-body signals.

Non-surgical TMJ care
Our TMJ care starts by resting the joint and balancing the bite, then adds therapies that calm the muscles and the inflammation around it. Where we can, we lean on therapies that support the body’s own repair, like light therapy and ozone injections, and keep medication to short-term help.
Orthotics, appliances and night guards
The TMD orthotic is a custom appliance that rests the joint and holds your bite in a balanced position so the muscles can let go. We adjust it over several visits as the joint settles. Where clenching is the main issue, a night guard might be enough.
Muscles, movement and daily habits
- Guided exercises: movements you do at home to retrain how the jaw opens and closes.
- Anti-inflammatory and muscle relaxant support: short-term help while the orthotic works.
- Physical therapy: we work with a physical therapist when neck and posture feed the problem.

Ozone injections and light therapy

When the chewing muscles stay overactive despite an orthotic, therapeutic Botox can relax them for a few months while the rest of your care takes hold.
How does sleep apnea get diagnosed, and what can a dentist do about it?
A dentist can screen your airway and treat diagnosed sleep apnea with an oral appliance, but obstructive sleep apnea itself is a medical diagnosis that requires a sleep study read by a physician. We’d rather say so up front.
The process in our office:
Screening
your CBCT scan shows where the airway narrows, and we go over your sleep symptoms.
Sleep study
if screening points to apnea, we coordinate a sleep study with an ENT or sleep physician, who makes the diagnosis.
Oral appliance
once mild-to-moderate sleep apnea is diagnosed, we make a custom mandibular advancement appliance that holds your lower jaw slightly forward so the airway stays open while you sleep. A few appliance types are available, depending on your bite and any TMJ findings.
If you’ve been prescribed CPAP and can’t tolerate it, ask us about appliance therapy as an alternative your physician can approve. Severe sleep apnea stays with the physician’s plan, and we’ll say so.

How long does TMJ treatment take?
Many people notice a change within the first few weeks of wearing an orthotic, though the timeline depends on how long the joint has been strained. A full course of orthotic therapy often runs several months with periodic adjustments. Injection therapy, light therapy, and physical therapy are scheduled as visits within that plan.

Not quite, since a night guard mainly protects teeth from grinding. A TMD orthotic is built from your bite analysis and scan to hold the jaw in a position that rests the joint and relaxes the muscles, and some people wear it by day. We’ll tell you which one your exam supports, since the simpler option is sometimes enough.
